Individual Medical Readiness monitoring and reports allow commanders and their medical support providers to track the medical readiness status of unit personnel, ensuring a healthy and fit fighting force, one medically ready to deploy.

IMR requirements are divided into six key elements: immunizations, dental, laboratory testing, medical equipment, periodic health assessments and Air Force Form 469 (Mobility Restrictive Profiles).

"The overall goal for a fully medically ready force is ideally 100 percent," said Master Sgt. Kimberly Bentley, 579th Medical Group. "The Air Force's more realistic goal is 80 percent. Airmen may be non-current for more than one element, but each Airman only counts as once non-current on the unit's IMR score."

Military members must be current in all six elements to be IMR ready. IMR requirements apply to all active and selected reserve members of the Air Force, including participating Inactive Ready Reserve Airmen.

Once in deployed status, members will no longer be monitored for IMR purposes during the deployment, plus one month following a deployment. However, in-theater medical units will continue to monitor members for vaccination status for force health protection purposes.

The U.S. Air Force Medical Services community uses the PHA Individual Medical Readiness data base to track IMR requirements. Public Health, Force Health Management Offices send commanders and unit health monitors medical readiness reports twice monthly. These reports spell out each unit's IMR requirements. Members are then notified and able to contact a primary care manager to keep medical readiness current.

For requirements completed at a non-Air Force facility members may request the single point of entry forms from 579mdg_sgpm@afncr.af.mil and send the forms back via e-mail or fax the forms to DSN 754-7128 or commercial (202) 404-7128.
